Honor leader awards will be selected from nominations received from 4-H members, leaders and parents. Awards will be distributed at the 4-H Leaders and Parents Holiday Party & Recognition Night in December. Please submit as much of the following information as you can:

• Your name and club affiliation
• Nominee’s name and club/project group
• Number of years of nominee’s leadership (best estimate is fine)
• Description of how nominee has contributed to the 4-H program.
